Form: FOI Request Container

This component provides a form for users to fill out an FOI request.

Usage Criteria

This component gives users a way to input and submit information needed to make an FOI request. It generates a fail state or a success state depending on if the information was submitted properly.

Do's and Don'ts

• Do not place this component at the bottom of a page below a large amount of content. The form should be visible without having to scroll too far down.

Design Notes

Because this component already takes up the majority of a page, it's good to limit the amount of other content that gets added with it. If content is added, ensure it is minimal and supplements the form.

Configurable Options

This component only requires a redirect link to a success page to be configured.
